Community Links Workers (6 FTE Posts)
The Community links worker programme is funded by the Scottish Government and delivered in partnership with GP Practices and the third sector to support people to live well through strengthening connections between community resources and primary care. We are looking to recruit 6.0 FTE Community Link Workers for Inverclyde who will each work in a GP practice, in an area of deprivation, as part of the practice team and provide 1:1 person-centred support to people in contact with a GP practice team. This post will work with individuals to enable them to identify personal outcomes and priorities for their health and wellbeing and link them to local and national support services and activities.

This is an exciting post at the cutting edge of collaborative working between the third sector and HSCPs in Scotland. The successful candidate will be working alongside both public and third sector organisations to improve services and outcomes for local people. Work will involve service redesign, commissioning, building collaborations and challenging partners.
The post-holder is jointly located at both the CVS Inverclyde and Inverclyde HSCP commissioning team.

It was part of his seminar on nutrition with the team at CVS Inverclyde, who are now working towards silver accreditation with Health Working Lives (HWL), an initiative that helps employers create a safer, healthier and more motivated workforce.
